CHF Southport Hosts Money Follows the Person presentation
Thursday, May 28th, 2009On May 19, 2009, Eileen Armstrong, Dir. of Social Services at CHF Southport coordinated a presentation for our Residents on the Money Follows the Person program in CT.
Dawn Lambert and Amanda Hampton spoke to an intimate group of our Residents about this program, and how it helps to transition nursing home Residents back into the community with the support they need. Literature was given to the Residents as well as an overview of the application process if an individual was interested in applying for the program.
